Introduction: Post-operative pain after any type of surgery can have very detrimental effects and if appropriate analgesia is not provided, it can effect respiratory, cardiovascular, gastrointestinal, urinary, and  endocrinological systems as well as have chronic effects like delayed recovery and chronic pain. In view of this, the present study was undertaken to study the effect of pain intensity and relief after application of transdermal diclofenac patch post-operatively on respiratory, cardiovascular, gastrointestinal system and associated complications if any. Material and methods: The study was conducted among 100 adult patients admitted in surgery department who went major surgery. Nupatch was changed every 24 hours postoperatively and patients were assessed for pain intensity and relief. Vital signs were observed at various intervals of time. Nausea, vomiting or any post-operative complications  if present were noted. Statistical analysis was carried using Chi-square test with p<0.05 as significant value. Results: The difference in values of pulse rate, systolic and diastolic arterial blood pressure in all the patients was found to be clinically non-significant. None of the patient complained of nausea after applying transdermal patch in the post-operative period. Similarly, no significant difference was observed in patients with relation to vomiting after applying Nupatch. Conclusion: The present  study showed non-significant changes on respiration rate, pulse rate, temperature and blood pressure as well as no complications and adverse effects were noted except skin irritation at various time intervals after giving transdermal diclofenac patch.
