ASSESSMENT OF EXTRA-PULMONARY AND PULMONARY TUBERCULOSIS IN PAEDIATRIC PATIENTS- A COMPARATIVE STUDY

Background: The impact of extra-pulmonary tuberculosis is greatest among infants and young children who tend to develop more severe extra-pulmonary disease, especially meningitis and military tuberculosis. Most tuberculosis cases in children are related to primary infection and mediastinal or hilar Lymphadenopathy, with a focal parenchymal lesion being the most common radiographic finding. This combination is helpful diagnostically when it occurs; however, some children do not have these findings. Hence; we undertook the present study to comparatively evaluate extra-pulmonary and pulmonary tuberculosis in paediatric patients. Materials & methods: The study included assessment of 100 pediatric patients was selected. 208 samples were obtained from 100 patients for analysis, including sputum and gastric fluid. All the samples were processed by standard decontamination before any procedure. Kinyoun acid fast-stain smears examination and Lowenstein Jensen cultivation and automatized culture were performed to all samples. The samples were identified to be positive or negative for Mycobacterium tuberculosis complex. The statistical analysis of the data was done using SPSS software for windows. Results: The most common signs and symptoms on admission were fever in 22 cases and cough in 33 cases. The incidence of cough was significantly higher in the PTB group when compared with EXPTB group. The number of patients without symptoms including fever, cough, malaise and weight loss were significantly higher in EXPTB (n=25) group when compared with patients in PTB group. Only 24 of the total patients had higher leukocyte counts while 59 of total children had increased CRP values. ESR was in normal range in 31 of the patients. Conclusion: In pediatric patients, diagnosis of extrapulmonary tuberculosis is more difficult in comparison to pulmonary tuberculosis.
